You will need to bring the following documentation to your appointment:
– Valid, government issued picture ID such as a valid passport (preferred), driver’s license or green card.
– Lists of current medication.
– Immunization records.
– If you had diagnosis and treatment for TB in the past, bring treatment record for TB.
– If you have had history of mental disease and treatment in the past, bring mental health records.
Please come 10-15 minutes prior to your appointment.
Yes, If you do not speak fluent English you must come with an interpreter.

Consultation for vaccines and vaccination are part of the initial appointment. Vaccines can be administered at my office, at a pharmacy (if you have medical insurance) or through public health. A referral letter for public health will be provided at your appointment.
MMR, hepatitis B, and varicella titer tests are available if you had those vaccinations before but do not have the vaccination record.
3-5 days if all lab results are negative. Additional 1-2 days if chest x-ray is required due to a positive TB test.
If referral treatment is required, then the time will depend on the length of the referral treatment.
After completion of I-693, you will receive a certified form I-693 in a sealed envelope.
Turn in the sealed envelope with your immigration application. A copy of the blood tests, results and exam will be provided for you to keep after results have been discussed with the doctor.
